Monitoring and Management of Long Term Conditions - Level 6 - AC6721

Current policy puts greater emphasis on keeping people healthy in order to help them live longer, illness free lives. This CPD module focuses on the biological, psychological, social and emotional impact of living with one or more long-term conditions and the strategies for monitoring and management which can be used.

You will explore person centred care, self-care and care planning as ways of supporting mental and physical wellbeing and you will have the opportunity to take advantage of specific sessions to develop skills in promoting health and supporting change. You will also be encouraged to critically reflect on your own skills, knowledge and clinical practice to identify areas for development and strategies for embedding monitoring and management of long term conditions into your practice.

Completing this CPD module will help you to develop a range of appropriate skills that will assist in the delivery of patient centred management of long term conditions.

  • History taking – frameworks for assessment

Previous evidence of level 5 study

1.  Define the concepts and underpinning principles of health promotion as part of the monitoring and management of long term conditions within a practice context.

 2.  Critically explore the attitudes, values and beliefs which may impact upon self-monitoring and management of long term conditions.

3.  Critically discuss the range of tools, specialist techniques and strategies which facilitate effective monitoring and management of long term conditions.

4.  Demonstrate awareness of and need for referral to specialist services if and when appropriate.
